Please briefly describe the initiative, what issue or challenge it aims to address and specify its objectives. (300 words maximum)
New Life Home in Pa-Phai Sub district, Lamphun Province, is a habitant for 2 groups of people; the disabled people affected from Leprosy and the general people. The Leprosy patients are fully cured from the disease; however, they developed disabilities. They are known as a fragile group of people in the society as they are limited to receive public health service and the accurate understanding of the disease which later caused low immune system and great risk of complication and dependency condition, resulting as a burden of the community and society The objectives are to provide higher quality of life physically and mentally to the people whose disabilities were affected from Leprosy by cooperating with every sector in assisting, sponsoring, as well as finding solutions for 27 disabled people and 7 affected people; to provide convenient and comprehensive public health services that are suitable to the context of the livelihood; to prevent further disabilities from Leprosy from entering dependency condition by educating the people about Leprosy; to support self-reliance with Sufficiency Economy Principles; to establish the care center for Leprosy patients for sustainability by applying proactive approach and 4S system as a mechanism which includes S1 = Survey: The survey is conducted by Family Care Team (PROFAST), S2 = Service: Fast and accessible service, S3 = Stop: Preventing dependency condition, S4 = Sufficiency Economy as the main philosophy

The initiative is in alignment with category 1, an equal and adequate service, since the aims are to ensure that the target group of the innovation, which is the disabled people from Leprosy who face more difficulties and delays in accessing public health service, is able to access the Empowerment of People process adequately and equally in both physical and mental areas. Moreover, to have every unit cooperate in solving problems, as well as to support the disabled from Leprosy to obtain suitable occupations and earn adequate incomes for living a life that follows the principles of Sufficiency Economy

Empowerment of people with disabilities from Leprosy with the 4S system is in alignment with the goal for sustainable development. Objective 3 ensures that the people possess good health in their everyday life and promotes the well-being of everyone at every age. Due to integrating all sectors in the use of databases from the community’s survey, the databased is used to improve the quality of life for people with disabilities from Leprosy both physically and mentally in order to stop the disability of people with Leprosy from reaching dependency condition Equal to 100 percent . Lastly, create good living conditions and promote the well-being of everyone in the community according to the principles of the Sufficiency economy, including the prevention of Leprosy

Create a participatory process for people in the community to search for problems and needs of the community; design problem-solving solutions together with the 4S system; integrating the working system of the government, the private, and the civil society sector; reduce complication of work between departments; able to solve the community’s problems continuously and efficiently; have a clear plan covering all dimensions including protection and support; establish the Leprosy service center in Pa Phai sub district; promote a lifestyle in accordance with the Sufficiency economy principles, and improve the environment to be hygienic

The Innovation is a form of Empowerment of people with disabilities from Leprosy who have difficulty accessing public services, especially in public health, and who lack caregivers due to the limitation of physical conditions that cause disabilities from Leprosy and complications. With the integration process of cooperation by families and communities joining in to set developmental goals and directions, there is a service center for people suffering from Leprosy. To jointly take care of the health of the disabled from Leprosy from becoming a dependency patient, one volunteer is assigned to take care of 10 households of the disabled from Leprosy and the affected people

By applying the 4S system community based to take care of the disabled from Leprosy due to the mechanism of work without restrictions of gender, social status, race, or religion. Everyone can access the service equally. This also creates understanding and awareness towards the community problems, as well as jointly finds solutions under the context of the community

The target groups benefited from the implementation are the disabled people from Leprosy and a number of 34 people who are affected in the community as the initiative was able to quickly bring supports and uplift physical and mental health to these people in New Life Home, as well as stop disabilities from advancing into dependency patients. Moreover, the affected people from Leprosy can acquire occupations and earn enough incomes for living

PaPhai sub district Municipality has set the guidelines for proactive work by studying the information which is a path to sustainable solutions. The cooperation with government sector, citizens, and various networks is integrated to implement help, support, as well as open the door for people to take part in thinking, making decisions, monitoring and evaluating the operation results. The implementation methods are as follow: 1. Storing the data, studying the problems and needs of the people with disabilities from Leprosy 2. Holding a meeting to propose ideas for finding solutions together for the disabled people with Leprosy, the residents of New Life Home, and related associations such as foundations, government sector, and civil society, etc. MOU in order to determine the ways to solve problems together. 3. Implementing Empowerment of People with disabilities from Leprosy with the 4S system with rapid access to information and rapid services by the 4S system which consists of S1 = Survey, a community-based survey with Family Care team (PROFAST); S2 = Service, a rapid and thorough service; S3 = Stop, prevention of dependency patients; S4 = Sufficient economy is a philosophy, applying the sufficiency economy principles in the lifestyle. 4. Monitoring and evaluating; must be able to specify the activities and how they are used for monitoring and evaluating. The committees for monitoring and evaluating are appointed from the representatives of the government and the public sectors in order to monitor and evaluate project with transparency. The meeting will be hold 2 times a year

The problems were the disabled from Leprosy’s fear of being disdained by the surrounded people and the limited interaction with only particular, which results in helping this group of people with slow progression concerning in health problems that are at risk of becoming dependency patients, the support for careers and incomes for living, by bringing the dimensions of society, economy and environment to be the drive for solving problems quickly and efficiently using the 4S system

Empowerment of People with disabilities from Leprosy by implementing the 4S system consisting of S1 = Survey: The survey is conducted by representatives of each family and volunteers of the village in order to refer the information to the public health center. S2 = Service: The public health service can be accessed quickly and easily. S3 = Stop: We can stop this disease from spreading when citizens can immediately access to the public health service. S4 = Sufficiency economy is a philosophy: there is the Promotion of Supplementary Occupations for the sustainable self-reliance

The innovation for the Empowerment of People with disabilities from Leprosy was re-initiated since 2017, by using the 4S system (fast track, fast service, fast prevention, self-reliance) under the integration of the cooperation from all sectors with community-based for sharing ideas, setting goals, implementing, granting benefits, inspecting and evaluating with the support from Pa Phai sub district municipality in order to solve the problems of the people effectively

It can be adapted for use in other contexts as there are still villages with disabilities from leprosy and the affected people live with normal people in the same way as new life villages. Which the government agencies Has allocated areas for patients who have been cured from leprosy and has another 2 villages, which are Tri Traksawarm Village, Ping Khong Sub-district, Chiang Dao District Chiangmai Province And Nam Phrae village, Nam Phrae Subdistrict, Hang Dong District, Chiang Mai Province By using community people as a mechanism to drive There is a system that is not complicated and requires a small amount of operation cost. Produce worthwhile results

The innovation for the Empowerment of People with disabilities from Leprosy was re-initiated since 2017, by using the 4S system which is adaptable for other contexts as the northern region of Thailand still has villages where the disabled and affected people from Leprosy live together with the general people in the same village, similarly to New Life Home. The government sector has allocated areas for patients who have been cured from Leprosy another 2 villages, Tri Sapawakarm Home in Ping Khong sub-district, Chiang Dao District, Chiang Mai Province and Nam Phrae Home, Nam Phrae sub-district, Hang Dong District, Chiang Mai Province, by using the people from the community as a driving mechanism with a simple system which requires a small amount of implementation cost and produces worthwhile results. Pa Phai sub district Municipality has shared the innovative knowledge of Empowerment of People with disabilities from Leprosy through various meeting platforms including 1. Knowledge sharing to the public through the community meeting platform 2. Knowledge sharing through the training platform for health volunteering leaders in Pa Phai sub district 3. Knowledge sharing through provincial meetings

Personnel - The representatives of the household select the people and looking after the household - Public health volunteers give advice on the protection and care from Leprosy; Public health department provides health services to people with disabilities from Leprosy and those affected 2. Materials - Orthotics for the disabled - Career supports materials - Database tools 3. Budget A budget of 50,000 baht for educational training, public relations media, career supports, Orthotics, and environmental adjustment in the livelihood of the disabled from Leprosy

Social dimension: Knowledge sharing and management by allowing the people to get involve; integrating the cooperation from both the government and organizations about the budget and materials which may result in sustainability Financial dimension: Service center establishment for people suffering from Leprosy with a revolving fund which is the savings of the group members in order to help the disabled people from Leprosy and those affected to develop their professions to earn incomes Environmental dimension: Promoting careers and incomes in accordance with the sufficiency economy principles by cultivating organic vegetables which are environmentally friendly and boost sustainable self-reliance

The deputy and the Prime Minister’s office with Best Financial Management Award 2019: The evaluating indicator includes Transparency indicators regarding public participation, public facilitation, professional management and the worthiness in using the resources, and people's satisfaction 2. King Prajadhipok's Institute and Provincial administrative organization with excellence in transparency and supports in public participation for the year 2018: The assessment tools include 1) Preliminary document evaluation 2) Assessment of public satisfaction 3) Group discussion assessment

The results of the evaluation found that the initiative could manage the resources in the area that are limited in terms of personnel and budget, resulting in worthiness and sustainability as follows: 1. An establishment of the service center for people suffering from Leprosy 2. A ratio of 1 public health volunteer per 10 households of the disabled from Leprosy and those 3. 100 percent of disabled people with Leprosy are able to access public health services conveniently that is suitable to the context of the area 4. A 100% prevention of the disabled from entering the dependency condition 5. A 100% of those affected by Leprosy received careers supports and earn incomes in accordance to the sufficiency economy principles and develop self-reliance

PaPhai sub district municipality has established a mechanism for public participation in the Empowerment of people with disabilities from Leprosy with the 4S system by integrating the cooperation between government, private and public sectors, by creating a MOU to help and solve the problems of the disabled from Leprosy and those involved in the community

The proactive work brings a real understanding towards the problems by using the 4S system (fast track, fast service, fast prevention, and self-reliance) as a mechanism to effectively drive the public services in the Empowerment of people with disabilities from Leprosy and those affected, resulting in a great achievement of the organization’s implementation
