Question 1
Please describe the objective of the initiative introduced (200 words maximum)
The government policy, according to the Global Plan for the Decade of Action for Road Safety 2011-2020 (The five pillars), particularly Pillar 5: Post-crash response, stipulates guidelines for emergency medicine development, timely and thorough coverage of services of treatment and rehabilitation of road accident victims. Songkhla Province has mobilized the policy according to the global plan and has defined it as a provincial agenda on road safety. As stated by the Emergency Medical Act B.E. 2551 (A.D. 2008), Section 33, Clause 2, Local administrative organizations are responsible for operating and managing local emergency medicine systems. Thus, Songkhla Provincial Organization has announced a code of law on emergency medicine, a local law that can be enforced sustainably, and has established an Integrated Emergency Medical Coordination and Dispatch Center with two main objectives: 1) to reduce the number of death resulting from emergency medical dispatch delays, and 2) to reduce the number of death while en route to the hospital. The aim of the initiative is to provide people with timely, thorough, and equal medical services 24 hours a day under the slogan “Develop People, Develop Work, Develop Services for People”

The initiative is aligned with Category 2. Administrators and related sectors have jointly created ideology and made strategic plans and formed policy, signed an MOU on cooperation in defining the roles and responsibilities, formed standard guidelines for admissions and referrals of emergency cases to reduce repetition of work; organized training continuously to upgrade staff members at the practical level and improve services standards, established an Integrated Emergency Medical Coordination and Dispatch Center with an interdisciplinary team to provide help 24 hours a day. Information is received and transferred through an emergency medical information system

The initiative is aligned with SDGs 3, KPI 6. Songkhla Provincial Organization, since 2000, has continuously conducted activities in solving problems relating to loss of lives resulting from accidents and emergencies. Under the project “One Rescue Unit, One Tambon/Sub-district”, administrators participate as committee members of the National Commission for Emergency Medical Services; this is obvious evidence of clear intention in mobilizing the emergency medical services system in Songkhla Province. In 2017, 100% emergency cases were saved with the pre-hospital emergency care system

In every province of Thailand, there is an Emergency Medical Dispatch Center in a hospital and the number is 1669 for timely medical services. To enhance the services, Songkhla Provincial Organization (SPO) has initiated an innovation for a new form of integrated cooperation by establishing an Integrated Emergency Medical Coordination and Dispatch Center No. 1669 consisting of Songkhla Provincial Emergency Medical Dispatch Center No. 1669, SPO CCTV Control Center and Songkhla Provincial Police Emergency Call Center No. 191. They are integrated to provide timely assistance for emergency victims and sufferers throughout Songkhla Province with efficiency. The team works closely with practical unit staff of local administrative organizations, foundations, and governmental and private hospitals. This initiative can provide confidence in life safety for locals and tourists resulting in better quality of life

Songkhla Provincial Emergency Medical Dispatch Center No. 1669, Songkhla Provincial Organization CCTV Control Center, Songkhla Provincial Police Emergency Call Center No. 191 are integrated under one emergency No. 1669 for emergency response team to rapidly arrive and provide on the scene care timely and correctly, and to efficiently provides care during the pre-hospital stage To optimize the speed of transport to hospital, the route is open with cooperation of traffic police and the CCTV control center. Information about the victim is sent to the hospital before arrival of the victim for medical preparedness
b. Please describe if the innovation is original or if it is an adaptation from other contexts (100 words maximum)
This initiative is a new innovation derived from work analysis which revealed that it was more efficient when the work of each unit or organization was integrated, and mechanisms were invented to coordinate the work to reduce steps in the work process for highest benefits. Resources are pooled to support and make operations more complete to achieve problem-solving at one stop. Victims are helped on the scene to reduce loss of life and risk of disabilities, and respond to people’s needs to the utmost

This initiative complies with the direction required by the Emergency Medical Institution of Thailand, and has received Emergency Medical Learning Space and Study Visit Venue Award. It is a training place for nursing students, and students of the Fundamental and Advanced Medical Practice Programs; it has been visited by many organizations, and a model for establishing a center of this type by several provincial organizations. Staff members of the innovation are invited to deliver lectures and speak in knowledge management sessions at a national level, and disseminate the initiative through various media

The emergency management is jointly conducted by local administrative organizations, governmental and private hospitals, all recue foundations in the province, and Songkhla provincial police under an MOU emphasizing decentralization, and networking. Guidelines for practices are formed corresponding with missions of each and all parties involved are informed. People’s potential is developed to be emergency responders. Ambulances and medical supplies are provided to cover all areas resulting in strong and sustainable teamwork synergy
b. Please describe whether and how the initiative is sustainable (covering the social, economic and environmental aspects) (200 words maximum)
The initiative has concretely integrated participation of the governmental, private sectors and civil society. Strong networks have been built to provide public services, the CCTV system is utilized to solve traffic problems and other emergencies. Pooled resources are shared to reduce steps of the process, save time, money, and workforce. There are strategic plans, work plans, financial regulations, knowledge management, and a related code of law for the work to be done continuously and sustainably as the work is still under each organization’s responsibility

1) The en-route mortality rate declined by 100% from that in 2016.
2) The percentage of critical emergency patients receiving emergency care in 8 minutes was 77.33 (The highest in the country).
3) The percentage of emergency patients reported through the emergency No. 1669 increased from 55.46 to 72.95.
4) The percentage of service recipients’ satisfaction towards emergency medical services was 92.40.

Participation of local administrative organizations, foundations, associations and the civil society sector is emphasized together with coordination, joint thinking, joint action, joint decision-making, and joint problem-solving, with Songkhla Provincial Organization as an administrative center for parties and networks involved. There are diverse activities for access and involvement in work system development such as networks meeting, opinions forums, line groups communication, follow ups and site supervisory visits, morning tea forums, and charitable activities

Please describe the key lessons learned, and any view you have on how to further improve the initiative (100 words maximum)
The jointly use of the information technology system has enhanced work in response to emergencies and preparedness for treatment. Local people are trained to be emergency responders for faster arrival on scene has resulted in community self-care and self-dependence, and task-based training has increased confidence among working staff and service recipients. Additionally, procurement of materials and equipment to support work covering all areas and determining the parking location of emergency ambulance at the center of service area can help to faster access and delivery services has resulted in goal-achievement quantitatively and qualitatively
